I've seen damaged and tampered boxes, but never bought one myself. I think that if TLG increased box security, the price would go up as well. If the problem stays small, They'll probably just keep sending out the missing pieces.

I think that they're all good. The major problems I've had is with the senders using inadequate packaging.
